Dr. Narwal started his dental career at the University of Manchester in 2011. Having graduated in 2016, he has since gained extensive experience in primary, community and secondary care settings across the country. He currently works as a Senior Resident clinician at EvoDental Bristol, providing oral surgery and implants for adults, and in practices near Oxford and Bristol.

He has experience in Oral and Maxillofacial Surgery and is therefore proficient in treating patients, who require advanced surgical intervention. He has also completed the SAAD qualification to evidence his sedation experience. He is currently enrolled in the MSc Dental Implantology programme at the University of Bristol, where he has achieved Distinctions in Years 1 and 2, in addition to receiving an award for being the highest scoring postgraduate student.

Dentistry is a career of life-long learning, and Dr. Narwal has consistently invested in courses to improve his skills, and remains keen to further his training in order to continue providing high quality, advanced care for his patients. He has presented his work internationally and has been published in dental journals. He also now mentors colleagues in oral surgery and implant dentistry.

His kind, gentle approach has often been recognised and his patients and colleagues have previously nominated him for 'excellence in patient care' awards. His core belief in dentistry focuses on good communication and empathy, with the patient being at the centre of each treatment plan.

Just another day in the office 🦷🔩

I've had an unconventional career. After DF1, I did 3 years of DCT with the intention to specialise in Orthodontics. However, having done a lot of oral surgery in secondary care environments, I realised that that was where my skillset and interest truly lay and I started to focus on oral surgery and implants in 2021.

Knowing that I needed to develop my restorative knowledge, I did a PGCert in Aesthetic and Restorative dentistry, which gave me a good foundation in treatment planning and execution of restorative dentistry. I then completed the Dentale implant course, which allowed me to place 25 implants under direct supervision. This gave me the confidence to start placing and restoring implants in practice. I also realised that there was a gap between my practical experience and theoretical knowledge of implants, so I enrolled in the MSc Dental Implantology at the Uni of Bristol.

The biggest growth came with my decision to join EvoDental. I've now placed and mentored over 1500 implants, which includes 300 full arch restorations.

Every year, I've invested a lot of my salary into my continuing professional development. In sum, I've spent over £150k on my postgraduate education over the last decade.

My decision not to pursue specialty training was also questioned by several peers, but my controversial opinion is that there are deficits in each of the training pathways if you want to focus solely on implants. Oral surgery and periodontics alone leave gaps in your prosthodontic experience, and prosthodontics leaves gaps in your surgical confidence. Restorative dentistry covers too many bases and doesn't focus enough on implants.

The best experience and training for dental implants is placing implants with appropriate supervision. Unfortunately there are few ways to gain that experience in the UK. The opportunities to gain that experience often require salary cuts that people aren't willing to make or investment into mentoring that is considered too expensive.

I don't earn the highest salary as an implant dentist because of these choices. But work doesn't feel like work. It's a joy and I wouldn't have it any other way.

Restoring aesthetics and function can be extremely challenging in Dentistry and when you combine that with a single central incisor tooth, the stakes get so much higher. ⁣
⁣
This pleasant patient saw me after fracturing a root at the front of his mouth and was not keen on a removable denture to fill this space. After deliberating between the two fixed solutions available, the patient opted for a dental implant. ⁣
⁣
After careful treatment planning, I was able to reduce the number of surgeries and combine the extraction of the fractured root, placement of the dental implant, jump gap bone augmentation and connective tissue grafting all in one visit. Chairside provisionalisation is a great technique to maintain the gingival architecture from collapse and resorption following extraction. ⁣
⁣
We had a discussion regarding replacement of the adjacent crowns to improve aesthetics and the patient is looking to explore this in the future. Both of us were delighted with the end result and now we will follow a focused maintenance plan to ensure long-term success!⁣

When this wonderful patient came to us seeking a fixed alternative to their long-term dentures, we knew we had a challenge ahead. Years of wearing a removable denture had accelerated bone loss, and natural aging and maxillary atrophy had further complicated the case. By strategically placing pterygoid implants and using a palatal approach in the premaxilla, we were able to avoid more invasive procedures like sinus grafts or zygomatic implants, reserving those options for the future if and when needed.

We see a lot of full smile makeovers on social media but patients might only have one aspect of their smile they don't like. For this patient, it was an unrestorable central incisor which had to be removed. To replace the gap, I placed an implant and restored it in a way to match his natural smile. He was delighted with the result as I met his brief of "making it look normal and not fake".

This lovely patient fractured their bottom molar which resulted in an infection which meant the tooth needed to be removed. They had a dental implant on the other side of their mouth which had been placed over 8 years ago thus she was keen for another dental implant. ⁣

The first photo highlights the importance of maintenance protocols to optimise long term results. It is a good habit to take a photograph of the correct sized interproximal brush that fits around posterior implants - firstly to educate your patients but secondly for our own documentation of maintenance protocols. ⁣

1. Do not always trust your CBCT. In this case, I was surprised to find a non healing large granuloma which left a sizeable bone defect at my planned osteotomy site. Instead of my planned drilling protocol, I changed my plan and underprepped my osteotomy leaning on the mesial wall to utilise 3 bony walls for my implant to lean on.⁣
⁣
2. Bone scrapers are great to harvest local autogenous bone from the ascending ramus. These autogenous bone chips are placed directly onto the exposed implant threads - in this case distal to the implant. ⁣
⁣
3. During second stage surgery (uncovery), try to map out the ideal flap size in an attempt to roll part of the flap buccally. In this case the final outcome was not affected however personally I found it tricky to manipulate a small flap around a healing abutment. ⁣
⁣

This patient lost their molar many years ago from heavy decay. They had always thought about replacing it and decided to see us for a consultation for information on the different options available. I discuss longevity with all patients, all ages as it is important they understand replacement events for implants and implant prosthetics. ⁣

This case had D1 bone so I prepared my osteotomy to the diameter of the implant with a coronal flare to reduce cortical pressure necrosis. By understanding bone quality during procedures, you can adapt your osteotomy protocols accordingly to achieve good primary stability.⁣
⁣⁣
I have carried out many cases guided, however this case was freehand (🧠 guided) with the appropriate radiographic checks to confirm correct direction and 3-D positioning.

This young patient had an unrestorable upper left premolar retained root which we removed. The patient explored all options for gap replacement and decided on a dental implant crown. ⁣
⁣
This was a tricky case as the mesio-distal space was incredibly limited at just below 7mm. This required precision on implant placement to avoid damaging the adjacent teeth. ⁣
⁣
One method of improving accuracy on placement is guided surgery. I have carried out many cases with guided surgery which have been brilliant and streamlined my workflow. In this case, I placed this implant free hand (brain guided) however I took pre operative radiographs with parallel pins to check I was on track. This step is crucial, as they say measure twice cut once!⁣
⁣

This was a huge decision to make as it is a sizeable investment, however when reflecting on this decision, I have no regrets. ⁣

I wore glasses from the age of 8 and then contact lenses from 16. Although both corrected my vision, there were situations which weren't so great (like when a contact lens falls out when you're out 🤣). ⁣
⁣
Since this treatment, I can enjoy waking up in the mornings being able to see, having a shower and even swimming with clear vision!⁣
⁣
It is important if you're considering similar treatment, to discuss your options with an Opthalmologist. You can normally get free consultations where you can find out if you qualify for the treatment and learn more about the benefits and risks of this elective surgery.

This pleasant patient could not tolerate a removable partial denture for their missing upper central incisors. As you can see from the photos, it was quite a tricky case to provide options for, as the mesio-distal space is too large for two teeth alone. It would have been very difficult to hide the retainers for a fixed bridge prosthesis. ⁣
⁣
After discussing the options, the patient opted for 2 single fixed implant crowns and accepted distal diastemas for correct crown widths in relation to the lateral incisor size. As the screw access channels were just on the incisal edge, we restored with cement-retained crowns for optimum aesthetics. ⁣
⁣
My main take home messages with this case were:⁣
⁣
1. Good photography is key for shade communication with your laboratory. ⁣
2. Verification jigs are very useful to locate your abutment correctly into the implant connection. For conical connections, this step is crucial! In this case we were working with an internal hex connection, so it was helpful to ensure correct seating in one of the 6 direction options.⁣
⁣

This lovely patient came to see me as she fractured her molar tooth which then developed an abscess. The tooth was previously root treated, and the patient did not want to go through root canal treatment again to save this tooth. After discussing her options, we decided to remove the tooth and replace it with an implant crown. ⁣
⁣
A few tips I've learnt from removing a heavily restored, root treated upper first molar:⁣
⁣
1. Decoronating the tooth but leaving 2mm+ of tooth supragingival is helpful to grip onto the root edges with your forceps⁣

2. After sectioning the roots, I find the disto-buccal root is the best root to remove first as it is straight. The mesio-buccal root is easier to remove after as it usually has a curved root which can then elevate backwards into that distal space ⁣

3. Always spend sufficient time debriding the socket with saline and removing granulation tissue. This step is crucial if you want to enter good quality bone during implant placement⁣

As we completed this case with a delayed approach, the healing was uneventful and the stages were very predictable. We also provided a bite raising appliance after treatment to protect her teeth and the implant because the patient is a bruxist.⁣

As @aspiredentalacademy say, we always want a healthy and happy patient. Good dental health is paramount, and heavily influences the success of dental treatment. That being said, I saw this lovely patient, who attended for a routine examination, and had optimal dental health with great oral hygiene. However, she was unhappy, with the appearance of her two front incisor teeth. ⁣⁣
⁣⁣
She had two composite veneers on these teeth approximately 7 years ago (which have done really well). After running through her options, she was keen to upgrade the composite veneers to porcelain veneers. In terms of her overall smile, she was happy with the alignment and colour. She did not want her lateral or canine teeth changing, just her central incisors. ⁣⁣

After providing her two new porcelain veneers, she was ecstatic with her new look, which she felt gave her more confidence to smile again!

This lovely patient attended after her upper molar fractured when eating popcorn, which was subsequently repaired in the short term with a temporary filling. When we discussed long term options of direct vs indirect cuspal coverage restorations, the patient was keen to protect the structure of her tooth with a porcelain crown.⁣
⁣
Following a clinical and radiographic assessment of her remaining dentition, we also detected tooth decay in the upper canine, and beneath the restoration of the upper first premolar. When dismantling the upper premolar amalgam and removing the tooth decay, we found crack lines underneath. The patient was then keen to proceed with a porcelain crown to proactively protect this tooth from fracturing in the future like her molar did. ⁣

Within two appointments we treated these three teeth, with a composite restoration, and two natural coloured pressed lithium disilicate crowns designed and manufactured by @alan_precisiondental 👌⁣

This was a case I learnt a lot from. This patient attended with a failing upper premolar that was unrestorable and had to be removed. They were unhappy with the space as it affected their chewing on the left side. Prior to the restoring this space, it is important to check if the underlying bone is well, the proximity of bone available to the maxillary antrum and any pathology associated with the adjacent teeth. In this situation, we had a distal bone defect, and an endodontic-periodontal lesion of the UL6, unsure whether it is primary endodontic or primary periodontal in origin. The root canal was carried out by an Endodontist, and I carried out a delayed implant placement with osseodensification using the versah burs. This allowed an internal sinus bump of 1-2mm and significantly helped increase the primary stability on placement. A synthetic bone graft and membrane were used to bulk the distal bone defect. ⁣The key parts to this placement was to plan prosthetically, and engage the mesial abundance of bone and higher sinus lining.

Now to wait 12-14 weeks before restoration!

Looking back at one of my favourite cases recently. This patient came in unhappy with their crowded bottom teeth and overall colour. They were happy with the shape of their teeth and did not want that changing. In approximately 9 months we were able to transform their smile with clear aligner therapy and external whitening. I love seeing an improvement in oral hygiene and gum health. ⁣

This month my wife and I decided to do the #veganuary challenge, based on the evidence that eating a plant-based diet is more energy efficient, and we've been thinking lots about the planet we're going to leave future generations. 🌿⁣

Harleen is already a vegetarian and not keen on meat/dairy substitutes, so most of what we ate was made at home with vegetables, tofu and legumes (pad thai, fajitas, pastas). But I found this month a lot easier than I thought it would be! I've been a pescatarian before, and vegetarian. But cutting out dairy in particular has helped me feel more energised and less bloated everyday. I also managed to lose 4kg of the extra weight I gained at Christmas (at a safe/healthy rate of 1kg/week), because it was harder to snack on unhealthy food. 💪⁣
⁣
There were some drawbacks. I had to be much more organised with my packed lunches, and I've also never read so many ingredients on packages before trying to eat something (and got caught out by honey a couple of times). 👀 But all in all, it's been a positive change and I'm happy to keep dairy out of my diet, and I'll be cutting back on meat considerably. ⁣
⁣
There are pros and cons of eating alternatives to meat/dairy (they're still processed foods), so do your research and make your own decision. But I'd really recommend trying the following when you're next eating out/ordering a takeaway/doing your grocery shop. I thought I'd share a case I see often presenting in general practice. If you're not afraid of blood, then feel free to swipe and see my approach. ⁣⁣

I would say there have been two phases in my learning of exodontia. The first, when I started dentistry. The second, when I started my journey with dental implants. Atraumatic extractions are key to preserving bone for patients, so you can safely plan a replacement tooth in the gap. One objective is not to lose the buccal bone, as replacement of this bone can result in unpredictable aesthetic outcomes. ⁣⁣
⁣⁣
I section nearly all molar teeth when removing them, in my current workflow. The benefits include time efficiency, improved healing for patients, improved comfort for patients during removal and (of course) reduced damage to the soft and hard tissues which is crucial when thinking about implant therapy. ⁣⁣
⁣⁣
In this case, I decoronated the existing crown. You can do this with a speed enhancing or surgical handpiece - in this case, however, the crown was relatively loose and came off gently with some forceps. Next I proceed with sectioning the two buccal roots straight across and then section the buccal roots from the palatal root. The key is to section down to bone, otherwise weak molars can splinter and present as more problematic. I find the MB root is commonly curved and can be difficult to remove first, so I normally take out the straight DB root first. This gives room to elevate the MB root with a mesial application point. Lastly I elevate the palatal root, with normally enough structure to remove with upper root forceps. This patient healed brilliantly and reported no post operative discomfort, and for myself it was a quick procedure in no longer than 30 minutes.

1 year today since our first vaccine dose!

Since then we've had our second doses, boosters, flu jabs, a Delta wave infection and an Omicron infection between us. A lot of people might wonder what's the point in getting the jab if you're still going to get covid - but neither of us got really sick with covid, or spread it to someone we care about. Definitely one of the best decisions we've made in 2021. 😷

Thought I would start of by sharing some cases as we enter work again for 2022. ⁣

This lovely patient came to see me after she broke her tooth. Ideally due to the loss of tooth substance, she would have benefited most from a cuspal coverage restoration however she opted for a large composite due to financial constraints. ⁣
⁣
Due to the lingual cusp being non functional with no opposing tooth, I made the decision not to reduce it and overlay. ⁣
⁣

I've been using my Oral B iO electric toothbrush for a few months now, and I can confidently say that it's not only the best Oral B toothbrush that I've used, but it's given me the best clean twice-a-day (that could only be beaten by a visit to the dentist/hygienist). 🦷

My favourite feature is the traffic light system - not only does it tell me if I'm brushing too hard and damaging my gums and enamel, but it also tells me if I'm brushing too lightly to effectively remove plaque. 🚥

Even though this was gifted, I've been wanting to get my hands on one for a while now, and it's definitely lived up to the hype. I'll be getting one for my wife next (as she's been getting jealous during our bedtime routine 🤣)

It's been nearly a month since we moved into our new home! 🏠 We've both rented in the past, so the next step was buying our own place. Over the past three years, I gained experience in a number of specialties in secondary care through Dental Core Training, with the goal to eventually specialise in orthodontics. My focus over the past two years had been to build my portfolio accordingly and prepare for entry onto a Specialty Training programme. I was unfortunately unsuccessful when I applied in 2019 and 2020 (another downer during the early days of the pandemic). Not many people talk about their failures - which is why I wanted to share this with you all. Lockdown gave me invaluable time to be introspective and explore what I wanted from my career and what to do next. Many of my mentors advised me to apply one more time in 2021, with the hope that I would get in on the third attempt. I realised deep down, I have developed some great skills in oral surgery and sedation and I would lose these skills, if I specialised in orthodontics. There were also personal factors to consider, such as the possibility of relocating to anywhere in the UK for the fourth time in four years (!). ⁣⁣

After completing 5 years of dental school and 1 year of practice experience, many early-career dentists take the leap to do something different and work in hospital. OMFS is dual-training specialty, so your seniors will have studied both dentistry and medicine. Juniors are usually only singly-trained in one of the two.⁣

Almost 5 years ago, a few dental friends and I travelled to South East Asia for dental voluntary work (Cambodia) and general travelling (Thailand and China) as part of an elective. This trip was organised more than 6 months in advance, hosted by the charitable trust One-2-One in the country Cambodia. This included organised voluntary work, accomodation and catering. ⁣
⁣
Dental voluntary programmes such as One-2-One are ideal for dentists, therapists and senior dental students. You are given the opportunity to do hands-on prevention and treatment, supervised and assisted by qualified local staff. In addition to this, you can enjoy the culture and food of Cambodia. A highlight was going to the beach every evening after a busy day of work and playing games with the local Cambodian staff. ⁣
⁣
Our main site of working was based in a local school where we relied on gym bench presses as dental chairs, a bin as a spittoon and a head torch for illumination. Our overall impression after was amazement at how obedient and well mannered the children were towards dental treatment. The language barrier was overcome by learning basic dental terms and assistance from local Cambodian nursing staff. ⁣
⁣
To any dental students who are thinking of undertaking similar voluntary work, I would highly recommend it. Even within 4-5 days I was able to extract approximately 50 teeth, and restore even more! This is a great confidence booster in terms of clinical experience in addition to your primary exposure at dental school. More so, you will gain one of the best personal experiences of your life - one that you will never forget! 🦷🌍

This often arises from referrals made from practitioners in primary care, who predict encountering or have encountered difficulties in delivering restorative treatment; however some may be referred from within the hospital from a different department.⁣

Treatment sessions in the core restorative sub-specialty (prosthodontic, periodontology and endodontic) clinics will be your bread and butter. You can also expect to see patients on consultation clinics where plans may be devised for the patient’s GDP to implement, or the patient may be accepted for hospital care if suitable.⁣
⁣
In the context of a DCT post in restorative; treatment sessions may be supervised or have a less formal element of supervision. It may become your responsibility to plan ahead and arrange for your diary to be booked in a specific manner, in order to facilitate supervision when doing tasks with which you are unfamiliar. Treatment may not always be as complex as you expect it to be; DCT is the platform for you to consolidate your skills and get the ‘basics’ done absolutely right.⁣
⁣
A DCT post in restorative may have a particular emphasis on a certain branch within restorative dentistry. This may result in your experience being different to another colleague’s, who may not have the exposure to the management of similar patients.⁣

🗣️ Discuss your confidence levels with your educational supervisor(s) for different procedures prior to the commencement of the year, to create a personal development plan (PDP) for you. PDP's can often feel like a tickbox exercise but they're actually a great way to assess your current level and formalise your goals. You might want to focus on surgical extractions, or improve your molar endo.⁣⁣

📖 Do background reading on NHS treatment, the UDA system, pricings and exemptions as this will be a new discussion you will be expected to have with your patients. Up until now you will have focused on your clinical skills; but you need to understand the financial side of things too.⁣⁣
⁣
📑 When starting, find out where referral forms, consent forms and prescriptions are kept so you have easy access when needed. Also read them to familiarise yourself and discuss anything you are unsure about with your team. I also downloaded the BNF app on my phone, and took screenshots of the drugs I regularly prescribed and saved them in a quickly accessible folder on my phone. 📲⁣⁣

👀 When you have spare time/a cancellation, take this time to shadow your educational supervisor or other associates. You might pick up alternative techniques and communication methods.⁣⁣
⁣
🦷 Practice a large range of cases (with as much time as you want to book in), and document them as they can help develop a nice portfolio for later in your career.⁣
